For Current Grantees

The Division of Planning will be collecting electronic submissions for routine forms. This is a voluntary option. If grantees choose to complete the form electronically, they do not need to submit a paper version. All paper forms must be scanned and emailed to sschroeder@hcnj.us.

Unless otherwise stated, current grantees must submit one of the following forms per grant by 5pm, Wednesday, June 30, 2022:

Status Report Form
This is the minimum requirement for all grants to remain in good standing. If a grantee was awarded multiple grants for the same project site, and no Commissioner action is requested, one form may be submitted with multiple grant numbers listed. Complete Electronically Download Paper Form
Extension Request Form
Grantees may request an extension for a grant expiring before October 2021. Each grant award should have it’s own request form. Complete Electronically Download Paper Form
Reprogramming Request Form
Grantees may request to reprogram funds to another project. Reprogramming of funded projects are granted at the discretion of the County. Only municipalities are allowed to request a reprogramming to a shovel-ready project within the same municipality. Shovel-ready projects are considered projects that have a selected contractor in place or are under construction. Reprogramming can only be awarded to Park Improvement projects and Historical Capital projects. No more than 50% of the total funded project award amount can be reprogrammed to a different project site. Each grant award should have it’s own request form. Change of Scope Request Form
